“Wiring the Behavioral Health Community”

BMHA established the Behavioral Health Community Resource Center (BHCRC) to convene healthcare information technology industry experts, population health and behavioral health (BH) providers to improve behavioral healthcare access and access to evidenced based chronic care management tools. The BHCRC goal is to transform the “point of care” patient clinical experience and make BH consultation and care accessible from multiple points within the community, not just through physician referral. Its centralized care coordination model features the BH Continuity of Care and Referral Coordinators (BH-COCRC) who link individuals referred from community-based sites to licensed, patient-centered BH clinicians in a virtual network. BMHA is uniquely positioned to grow an interactive e-database of culturally competent and patient centered licensed behavioral health clinicians. The goal is to improve outcomes, reduce cost and enhance the patient’s experience and overall satisfaction.  BMHA Strategic Partners include:
